Introduction and outline of the workshop - Sally Savage (SS)

Three short presentations (10 minutes each) will highlight key management issues that will be considered in the case discussion component of the workshop.  The workshop will be designed to challenge, provoke and engage the audience.

· Who are older people? - Trisha Dunning (TD)

· Caring for older people with diabetes: nursing and social perspective - Nicole Duggan (ND)

· Caring for older people with diabetes: a medical perspective - Dr Mark Kennedy (MK)

· Discussion (10-15 minutes)

Interactive case discussion: The discussion will show common care trajectories and the transitions older people and their families negotiate.  The case history will involve interactive discussion from participants and will be presented in stages that step through the individual’s changing care needs as function changes.

· Functionally independent

· Functionally dependent in some ADLs – family assistance needed but what if that is not available?

· Cognitive changes

· Admitted to a care home: key challenges providing care for older people with diabetes in care homes

· Carers’ needs: family and health professional

Possible discussion issues:

· Need to involve the individual/family in planning care.

· Atypical symptoms for key issues such as new onset diabetes, hypoglycaemia, hyperglycaemia, UTI and pain.

· BG, HbA1c and other targets.

· Nutrition: the myth of the ‘diabetic diet,’ under- and malnutrition and their consequences.

· Managing medicines:  PIMs, AIMs, AE risk, considerations with the main types of GLM.

· Key assessments e.g. ADLs, cognition, frailty, Palliative score, pain, hypo risk, medicine risks

·  Education: effect of changes in memory and learning and the way older people acquire, code, store and retrieve information and sensory deficits such as vision and hearing loss.

· Discussing palliative and end of life care.
